Annual Facility Reporting Information

NEW: 2016 Forms and Instructions are now available. Please scroll down to the 2016 section.

NOTE: The Annual Facility Report (AFR) must be postmarked no later than March 1st  of every year covering activities conducted the previous year. Please, DO NOT fax completed reports.

Attention: Hazardous Waste Generators are NOT required to file the Annual Facility Report. If you are not a Treatment, Storage, and/or Disposal Facility, please click here to go to the Biennial Hazardous Waste Report page.

The Department of Toxic Substances Control holds facilities that handle hazardous wastes to high standards of operation. TSDFs are required to report all RCRA and State hazardous waste activities in order to maintain regulatory compliance in accordance with the AFR.

DTSC collects and maintains information via the Annual Facility Report to analyze hazardous waste management activities in the State. DTSC uses the data to identify trends in hazardous waste management and to monitor hazardous waste facilities in California.

A Treatment, Storage, or Disposal Facility (TSDF) is required to file the Annual Facility Report (AFR) pursuant to the California Code of Regulations, Title 22, Division 4.5:
