NOTE: The Annual Facility Report (AFR) must be postmarked no later than March 1st of every year covering activities conducted the previous year. Please, DO NOT fax completed reports.Attention: Hazardous Waste Generators are NOT required to file the Annual Facility Report. The Department of Toxic Substances Control holds facilities that handle hazardous wastes to high standards of operation. TSDFs are required to report all RCRA and State hazardous waste activities in order to maintain regulatory compliance in accordance with the AFR.
A Treatment, Storage, or Disposal Facility (TSDF) is required to file the Annual Facility Report (AFR) pursuant to the California Code of Regulations, Title 22, Division 4.5:

Submit the AFR electronically using the web version, EASITRAK. Print, sign, and mail the Certification page (fourth page of the SI Form) and the Declaration of Electronic Filing Form to:

FREQUENTLY ASKED QUESTIONS (FAQs)
Is there a fee to file the Annual Facility Report?
No. There are no fees required. However, failure to comply with State regulations regarding Annual Facility Reports may result in fines and penalties.
Do you mail courtesy notices?
DTSC is not sending a courtesy reminder notice to TSDFs this year.
Do I need to file an Off-Site Identification Form with my Annual Facility Report?
No. California does not require the Off-Site Identification Form.
